Many of my patients feel that as they get older, their eyes appear smaller, more tired, and more wrinkled. For both both genders and all ages, the goal of eyelift surgery is to give you natural results that make you look younger, fresher, and more alert, without obvious signs of surgery.

This is a sampling of real, achievable results from blepharoplasty (eyelid surgery, or eyelift) amongst my actual patients. Although of course everyone is different, and surgical techniques and patient anatomy vary, here are some things that I focus on trying to achieve for each blepharoplasty patient:

  • hidden incisions
  • camouflaged scar lines that are nearly imperceptible
  • enlargement of the appearance of the eyes
  • a less tired first impression
  • a more youthful first impression
  • preservation of normal eye function and anatomy
  • respect of the periorbital tissues and anatomic landmarks around the eyes
  • the highest degree of patient satisfaction

One major worry that patients have when thinking about eyelid or periorbital surgery is about their scars. Many people are concerned with looking overly "done" with tell-tale signs of plastic surgery.

My approach to eyelift (blepharoplasty) surgery in my New York City private practice is one that takes great care in ensuring that all scars are hidden, either inside the eye, at the edge of the eyelash border, or both. This gives patients a durable, natural result without visible scars.

Shown below are two male patients of mine, each with different concerns, but both with invisible scars and natural results.

The top patient (above) is in his 20s, with excellent skin quality and almost no wrinkles. His main concern was dark circles, tired-looking eyes, deep tear troughs, and puffy undereye bags. Because of his youth and good tissue quality, I performed his surgery with entirely hidden incisions on the inside of the pink part of his lower eyelid.

The bottom patient (below) is in his 50s, with significant wrinkles and less elastic skin quality. He was concerned with looking older, tired, and wrinkled around his eyes. In this patient, I performed upper and lower lid eyelid surgery with incisions hidden in the eyelash margin on the bottom and in the natural lid crease on the top.

Male Abdominoplasty (Tummy Tuck) Surgery

Most people think of the "tummy tuck" or abdominoplasty surgery as a women's operation. Indeed, it is more common in women, likely because women undergo more weight fluctations due to childbearing, and often experience disruption of their abdominal muscles.

However, abdominoplasty in male patients is a powerful technique for restoring an aesthetic contour to the trunk, and patient satisfaction is extremely high. Men who have scars from previous abdominal surgery, hernias, or excess skin and fat due to weight loss benefit significantly from this technique.

When performing an abdominoplasty in a male patient, I design my incision in a different manner, to create a more masculine, triangular-shaped torso. (In women, the incision and technique is designed to accentuate an hourglass figure).

Beautiful eyelashes go with everything-- they brighten up the face and make the eyes look pretty, flirty, and feminine.

The above images show actual patients before and 16 weeks after using Latisse, a prescription-only serum that lengthens and thickens the eyelashes. These women are not wearing makeup, eyeliner, or mascara in any of these photos.

Latisse is a liquid that is applied to the upper eyelash margin with a tiny paintbrush once a day. It begins to work as early as one month, but full results are evident at 4 months. Abdominal Liposuction for Refinement of the Six-Pack

Many patients who are already fit desire additional contouring of their physiques to look their absolute best in outdoor, athletic, beach, and intimate situations. For patients in this category, liposculpture, or abdominal liposuction for refinement of the six-pack can be an excellent option.

Depending on your unique anatomic concerns, surgery takes approximately 1 hour, and anywhere from 1 to 5 Liters of fat lipoaspirate can be removed. Incisions (including the main incision, which is hidden inside the belly button) are intentionally camouflaged, and typical incisions are no more than half a centimeter long.

After surgery, you wear an abdominal binder (a tight garment that keeps firm pressure on your midsection) and resume your normal activities when you feel ready. Many patients are able to go back to work in 1-3 days. Swelling takes time to resolve, but by 2-3 weeks after surgery, you will already begin to see your results. Every week thereafter, you will note additional improvement, until about 6 months, when all of the swelling is gone.

Demystifying labiaplasty

Cosmetic rejuvenation of the female genitalia has been a taboo subject for so long that speaking about it openly and accurately remains a challenge, even within some segments of the medical community. I have had labiaplasty patients tell me that their OB-GYN or internist told them that nothing could be done to help them. I find this disheartening.

Can you combine breast and body surgery?

Many body contouring patients in my practice are interested in multiple operations. The most common areas of concern are the breasts and the abdomen. This makes sense, as these two anatomic regions tend to bear the brunt of weight gain and loss, pregnancy, breastfeeding, and gravity.

How do we decide whether or not operations can be combined? The main factor in determining whether or not breast and body surgery can be perfored in the same setting is patient safety. Consideration of the amount of time required for a given operation, an individual's medical history, and the surgical goals of a particular operation are all factors.

Plastic surgeons typically try to keep elective operations under 5-6 hours in length. In my practice, I prefer to keep total operative time under 3.5 hours. Research shows that shorter operations have a lower risk of infection, blood clots, and anesthesia issues. In my practice, this is paramount.

Within this framework, I am able to combine breast and body operations in select patients, as in the weight loss patient above, who underwent breast lift (mastopexy) and tummy tuck (abdominoplasty) with excellent results.

Why Are So Many Teenage Girls Getting Genital Cosmetic Surgery?

Much like a snowflake, no two vaginas look exactly alike; there's a wide range of variation in terms of the vulva's external appearance, and it's all totally normal. Unfortunately, most young women aren't taught about the diversity of vaginas in sex ed, which means that some might opt to go under the knife to get the perfect vagina

The New York Times reported the number of teenagers seeking labiaplasties (a surgery that reduces the size or shape of the vulva) has gone up by 80%, with 400 young women under the age of 18 undergoing the procedure in 2015, as opposed to 222 in 2014. Last week, the American College of Obstetricians and Gynecologists released a statement urging doctors to screen for disorders like body dysmorphia when young women request the procedure.

"When adolescents seek medical treatment, the first step is often education and reassurance regarding normal variation in anatomy, growth and development," the statement said.

Labiaplasty has become one of the country's fastest growing cosmetic surgeries, with the number of overall procedures rising 49% in 2014, according to a report from the American Society for Aesthetic Plastic Surgery. Although young women age 18 or younger currently only make up 4.6% of all labiaplasty patients, the trend among young women is continuing to surge.

There are, however, some potential complications associated with the procedure. 

"The big thing I tell patients about labiaplasty is that there are a lot of unknowns,"  Dr. Julie Strickland, chairwoman of ACOG's committee on adolescent health care, told the Times. "The labia have a lot of nerve endings in them, so there could be diminishment of sexual sensation after surgery, or numbness, or pain, or scarring." 

On a broader cultural level, there's a need to educate young people about the natural diversity of vaginas. The prevalence of porn has led to many women believing that vulvas are supposed to be trim and compact, leaving them feeling inadequate if they fail to measure up. 

A psychological study by Australian researchers published by Oxford University Press in February surveyed 351 women and found a direct correlation between media consumption and labiaplasty inquiries. 

"Women seeking labiaplasty have been exposed to a greater volume of these idealized media images," the report said, "and display a stronger desire to resemble these images than women not seeking labiaplasty." 

Yet not all medical experts are concerned about the growing popularity among young women. 

Cosmetic surgeon Dr. Lara Devgan said in a phone interview that she regularly performs labiaplasty on patients between the ages of 17 and 19 and calls it a "very safe surgery with high satisfaction rates." She said she receives around five inquires about labiaplasty a week, between 20% to 25% of which are from women under the age of 25.

Most women, Devgan said, seek her services because they want to feel more comfortable with the way their genitals look. A small number of young women report experiencing constant discomfort as a result of having large labia. 

"Some women tell me they have to adjust themselves several times a day," she said. "It [labiaplasty] changes their lives." 

Devgan said it's a misconception that the rise in labiaplasty stems from the popularity of online porn or women feeling inadequate in the bedroom. "Most of my young patients come in with their mothers and aren't sexually active. They are doing it for themselves," she said.

While that might very well be the case for some young women, the surgery comes at a steep price. Devgan estimated it can cost anywhere from $8,000 to $15,000, depending on the doctor and how many areas of the vulva are altered, although the national average is closer to $2,700

But as long as the patient is past puberty and has parental consent, Devgan doesn't see any problem with young women seeking the surgery for purely cosmetic reasons. She said she believes the conversation surrounding labiaplasty has, for the most part, been beneficial for women.  

"For so long there's been a stigma about women's bodies," Devgan said. "Finally people are able to talk about what's been bothering them for ages."

3 approaches to chin augmentation

People rarely think about their chins, but chin projection and proportion are very important in facial aesthetics. Having a chin that is in harmony with your bone structure, jawline, nose, and face shape can mean the difference between having an attractive face and an ordinary one.

Chin augmentation is a general term that describes a range of procedures that includes injectable chin filler, chin implant, and osseous genioplasty. The images in this post are some examples of my actual patients who have undergone these different techniques.

Chin Filler

Injectable chin filler (typically with Juvederm, Voluma, Restylane, or Radiesse) is a minimally invasive, non-surgical procedure that takes about 5 minutes. Under sterile conditions, an FDA-approved product is injected into your menton (chin prominence). Results last 1-2 years, and chin projection and contour can be made very attractive.

Chin Implant

A chin implant is a soft solid silicone or Medpor device that is surgically placed via a tiny incision hidden under your chin crease or inside your mouth. It is typically done with light sedation ("twilight"), although it can be done under local anesthesia with a nerve block. Results are permanent and very attractive. In my opinion, this is the gold standard best technique for chin augmentation.

Osseous Genioplasty

Osseous genioplasty is a surgical technique for repositioning the bony prominence of the chin and holding it place with tiny titanium plates and screws. The incision is hidden inside the mouth, and results are permanent, although the recovery and healing process can be quite involved due to the pain of bony surgery.

Labiaplasty is a popular aspect of my New York City private plastic surgery practice. The reasons women seek labiaplasty are varied and include:

  • dissatisfaction with the aesthetic appearance of the labia minora, clitoral hood, labia majora, perineum, or some combination of those
  • discomfort with sports or exercise
  • difficulty with hygiene
  • the frequent need to adjust the private parts
  • self-consciousness when in intimate situations or fitted clothing

Every labiaplasty I perform is uniquely tailored to each patient's specific concerns and one-of-a-kind anatomy. No two labiaplasty patients are the same, and no two operations are the same either.

I am well-versed in the full spectrum of labiaplasty repair techniques, include wedge excision, lazy-S excision, en bloc reduction of labia minora and clitoral hood, and others. The important aesthetic points for each operation include:

  • matching the pigment borders and lines so that the pink pigmentary line and beige/ brown pigmentary line are continous and aesthetic
  • ensuring no rippling or puckering of the incision borders
  • hiding scars as much as possible
  • achieving the best possible symmetry given a patient's anatomic starting point

The key to achieving the most perfect possible results for a labiaplasty surgery lies in planning, meticulous attention to detail, and stepwise refinement of excised specimens (as pictured above). In this era of gynecologists, general doctors, and high-volume "labiaplasty centers" performing these surgeries, it is important for women to understand that a board-certified plastic surgeon is trained in the delicate tissue handling, aesthetic principles, and surgical anatomy that give you ideal results.

The Golden Ratio As Applied to Facial Beauty

Some say if you draw a line vertically down the center of your face and the left side is seemingly, symmetrical to the right, then you have what is defined as a scientifically beautiful face. In fact, if you are curious to what you would look like with a perfectly symmetrical face, find a mirror and hold it perpendicular to the center of your nose. Lining the mirror at the center of your hairline, along your nose, and the center of your chin, you can get an idea of what you would look like as your most scientifically beautiful self.

Though a vast majority of the world can safely say they do not have a deceptively, symmetrical face, there is still hope for some of us. Dr. Stephen Marquardt conducted a study over several years that ultimately revealed the “Golden Ratio” of beauty. The Golden Ratio uncovers 23 areas of the face that are measured on a scale of phi, 1.618. According to this ratio, the more of the 23 defined areas that line up with this gauge on your face, the more beautiful you are. Along these same guidelines, Dr. Marquardt found that this scale measures beauty of all ethnic backgrounds and both females and males. This led him to originate a “beauty mask” that uses a pentagon and decagon as the cornerstones that ultimately lead to the same phi, 1.618, Golden Ratio.

This standard of beauty can be daunting to even the most beautiful of people. When writing about the Golden Ration, writer Gary Meisner, noted that there are infinite versions of beauty that lie outside the lines of the mask that Dr. Marquardt has created. It is important for all women and men to feel that they are not defined by a set of standards.

With this being said, when considering any altercation of your look with surgery or procedures such as filler and botox, it is important to keep in mind your overall facial structure. With any procedure, Dr. Devgan studies all unique characteristics of the face and uses a mastered technique to give her patients their desired outcome while still maintaining a natural look.

How to find a bra that fits

Breast surgery (breast lift, breast augmentation, breast reduction, and inverted nipple correction) is one of the most popular aspects of my New York City practice. I spend a great deal of time getting the technical details of surgery precisely right, to ensure that my patients are pleased with the way they look when they take off their surgical bras. When it comes time to buy a new bra (whether or not you have had surgery), here is some advice from my office to get a bra that fits you properly, feels comfortable, and flatters your appearance. If you want your breasts to look the way the do when your bra is on (even when you take off your bra!), you may be a candidate for breast implants or mastopexy/ breast lift surgery. -Dr. Devgan

How a bra should fit

Getting a new bra can be an exhilarating feeling that almost all women can relate to. But, how many of us go into our favorite lingerie store, find our size, and buy a bra without trying it on? When we go to wear the bra for the first time we find ourselves tightening the straps as tight as they can go or as loose as they can hang and cinching the back to its tightest eyehooks. After speaking with a sales associate at a local high-end lingerie store, I was astonished to find that this is not how a bra should fit, nor do any two bras fit the same.

What are breasts made out of?

Breast tissue is composed of glandular tissue (which is responsible for milk production), fatty tissue (which can fluctuate depending on your body fat percentage), and ligaments (which hold the breast up in a perky position). Cooper's ligament, the main structural supporting ligament for perky breasts, is easily stretched due to regular movement as well as gravity, leading to drooping of breast tissue over time. 

Not wearing an appropriate bra can eventually lead to more than just unattractive physical appearance. It can also lead to bad posture, neck, back, and/or shoulder pain. Please take this into consideration the next time you are going to purchase a bra. Make sure you are getting properly measured and making the appropriate adjustments as your bra wears out overtime.

It’s time to take the right steps to finding a good bra.

First, know the important, yet basic, components of the bra:

  • The back strap (a.k.a. the wing/backbend measurements)
  • The shoulder straps
  • The cups

Next, know how each of these components plays a part in a well-fitted bra.

The wings, or backbend, are where most of your bra support comes from. Once the bra is on, the back should be hooked on the last eyehooks. The last eyehooks are the ones that leave the bra fitting the loosest. This way as the bra stretches over time, you can tighten as necessary.

Then there are the straps.

The straps also play a large roll in overall support.

Straps should only be tightened to lay snuggly on the shoulders. If the straps are digging into the skin on the shoulders or the backbend of the bra appears to be lifted, or higher then the cups of the bra, then the straps are too tight. A brand new bra should never have the straps at their tightest point. Eventually, a bra begins to stretch and the rings and sliders on bra straps are made primarily for tightening overtime.

Lastly, consider the bra cups.

When determining what cup size you are, there are a lot of factors that can weigh in: padded vs. unpadded, pushup vs. full coverage, underwire vs. unwired lining, and even lace vs. cloth. The most important thing is to remember your entire breast should be able to fit inside the cup, with the nipple at the center, and the apex, or top of the cup, should lay flat against your skin. If it is indenting into your skin, or the side of your breast, near the armpit is hanging out, you need a bigger cup. If there is room between your skin and the bra at the top of the cup, then you need a smaller cup.

Facts and Myths about Liposuction

Liposuction is one of the most popular plastic surgery procedures in America. It is a minimally invasive surgery that literally sucks out extra fat in unwanted areas. Whether it is in your arms, thighs, belly, above your knees, or even your neck, liposuction is one of the most powerful surgical techniques for getting rid of unwanted fat.

How does liposuction compare to Coolsculpting?

Liposuction is the gold standard technique for removing fat. Its efficacy far surpasses some of the more "trendy" surgical fads of the moment, including Coolsculpting, Ulthera, Thermi, and others. Not only does liposuction have better, more pronounced, and more durable results, but the conceptual basis for liposuction is irrefutable: fat is physically being removed from the body to create a leaner contour.

What is liposuction?

Liposuction is a surgical procedure that uses cannulae (thin surgical straws) that are hooked up to a vacuum suction device to suck fat out of stubborn pockets of chubbiness on the body. A special numbing solution (tumescent fluid) is used to ensure a comfortable recovery with almost no bleeding.

What is the recovery like?

Generally speaking, liposuction has one of the quickest recovery times among plastic surgery procedures. Many people return to work within a day or two after surgery and begin light exercise as soon as they are comfortable. The incisions made are very small and aesthetically appealing, as they are hidden in natural creases of the body. Because of its relative lack of invasiveness, liposuction can easily be combined with other surgeries such as a tummy tuck, breast reduction, breast lift, breast augmentation, facelift, necklift, and many more.

When are final results apparent?

Despite the relatively little downtime, it is important to note that bruising and swelling from liposuction can last for several weeks. Use of compression garments is recommended to reduce swelling and improve results. Most patients note improved contour and appearance within 1-3 weeks after surgery. However, some degree of edema can be present for 6-12 months before final results are fully appreciated.

What are the limitations of liposuction?

Though liposuction is a powerful operation for contouring the body, it is important to remember a few things.

  1. It is not a weight-loss tool, nor is it a treatment for obesity.
  2. The more elastic your skin is, the better your results will be. If your skin is loose and inelastic, you may need a more definitive procedure (such as a tummy tuck, arm lift, or neck lift) to avoid having saggy areas of skin after liposuction.
  3. Liposuction is not meant to remove all of the fat in your body. Instead, well-executed liposuction removes just the right amount of fat to contour the body into a natural, yet lean, shape.

How does Dr. Devgan create beautiful results with liposuction?

Though liposuction sounds simple, bad liposuction (with asymmetry, contour abnormalities, divets, notches, and dents) is very common. Dr. Devgan uses a variety of expert techniques to try to give each of her patients the best possible results. These include:

  • use of a variety of cannulae in the smallest appropriate gauges
  • use of a cross-hatching technique
  • precise counting of liposuction passes and strokes
  • use of tumescent or superwet technique to minimize blood loss and discomfort
  • use of thorough preoperative marking and photographs to map out a successful liposuction result before the surgery has even begun
  • double and triple checking each patient for symmetry, smoothness, contour, and appearance from multiple angles
  • placement of incisions in hidden locations in body creases
  • delicate suture closure of all incision sites

What are my next steps?

If you are thinking about liposuction for body contouring, there are several things you can do to optimize your results. First, try to get to your goal weight via healthy lifestyle choices including a health-conscious diet and exercise regimen. Second, try to maintain an adequate degree of muscle mass to help with mainaining your healthy, lower weight. Third, make sure your weight has reached a plateau.

Breast augmentation with internal lift (no scars on the breast)

When I perform a breast lift (mastopexy) in my New York City practice, many patients are concerned about having scars on the front of their breasts. A traditional breast lift requires either a donut (periareolar) scar, a lollipop (vertical) scar, or an anchor (Wise pattern) scar. All of these are effective for different patients, but each of them carries the downside of creating a scar on the front of the breast.

I use a technique for breast augmentation with an internal lift that allows me to give my patients a modest breast lift of about 2 centimeters, without placing any scars on the breast. This technique doesn't work for every patient because if you have a significant amount of droopiness, you will need one of the other techniques. However, for the right patient, this is a great way to get a beautiful result without obvious scarring.

What age is the right time for Plastic Surgery?

Patients are often curious when they should have a plastic surgical procedure. There is no magic time frame that is right for everyone, and there is no correct answer to that question. Plastic surgery is highly individualized and depends on a variety of factors, including physical maturity, emotional maturity, readiness, and optimization of overall health.

There is a wide age range for people who undergo elective plastic surgery. From teens who are growing into their bodies to seniors who want to reverse signs of aging, plastic surgery has varying prevalence across age groups. Though many procedures can be performed at any age after puberty, data published by the American Society of Plastic Surgeons reports the following plastic surgery trends by age group:

  • Rhinoplasty – ages 13 to 19

  • Breast Augmentation – ages 20 to 29

  • Liposuction/body sculpting procedures – ages 30 to 39

  • Eyelid (blepharoplasty)/eyebrow lift – ages 40 to 45

  • Facelift and necklift – age 55 and older

What does this say about your age group?

RHINOPLASTY (ages 13–19): The nose changes the least out of all of the facial features, and it is often a cause for concern starting in one's teens. I advise patients to wait until the craniofacial skeleton has finished growing (typically around age 15 in girls and age 17 in boys) before thinking about rhinoplasty or nose job surgery. This allows your surgical result to match your facial features without causing any disturbances in facial growth. Surgery in minors always requires the consent of a parent or guardian.

BREAST AUGMENTATION (ages 20-29): Once puberty is complete, a woman can assume her breasts are fully formed. While I have performed breast augmentations in teens as well as women in their 70s, the twenties and thirties seem to be the most popular time for first exploring this procedure.

LIPOSUCTION/ BODY SCULPTING (ages 30-39): Usually by the time both women and men are in their thirties they have begun to go through metabolic changes. When exercise and a healthy diet no longer suffice in getting rid of stubborn body fat, people begin turning to different forms of liposuction and body sculpting techniques. These surgical procedures have proven to safely and successfully remove fat.

EYELID SURGERY/ EYEBROW LIFT (ages 35 to 45): Men and women’s eyes typically begin to show signs of aging in their thirties and forties. The periorbital skin is the thinnest on the body, and genetics, sun damage, time, and stress can reveal themselves in the form of wrinkled skin, excess tissue, puffy bags under the eyes, and dark undereye circles. A subtle way to give aging eyes a renewed look is upper and lower blepharoplasty (eyelid surgery). Blepharoplasty is one of my favorite, signature procedures, and one of the most popular cosmetic surgeries in America.

When skin around the eyes and on the forehead begins to sag, it can also cause eyebrows to droop. Depending on each person’s unique anatomic characteristics as well as their desired look, either a non-surgical or surgical option can be offered to give eyebrows a refined raise.

FACELIFT/ NECKLIFT (ages 50 +): From the forties, fifties, and onward, many people realize that parts of their face and neck are not exactly how they remember them. Sagging skin can cause more prominent nasolabial folds, loss of definition in the jaw line, and loose, hanging skin on the neck. When done correctly, a facelift and necklift are incredibly subtle operations that can have significant and dramatic effects in improving a person's overall appearance.

So what is the best time for plastic surgery?

In my New York City practice, I see patients at all points on the age spectrum, from childhood to older ages. The right time to have surgery is different for every person, and there is no single cookie-cutter mold for every single patient. If you are physically healthy, emotionally stable, and concerned about a particular feature of your face or body, seek the opinion of a board-certified plastic surgeon to learn more about your options and the best time for your potential procedures.

Gummy bear breast implants vs silicone implants: What do women in NYC prefer?

One of the big decisions with breast implants is what type of implants you will get. In this day and age, over 90% of the patients in my New York City private practice opt for silicone implants. This is because silicone implants tend to look and feel more natural than saline implants. In addition, in thinner women, silicone implants are less likely to demonstrate visible wrinkles or folds.

Within the category of silicone implants, women have the option of choosing "gummy bear" silicone or traditional highly cohesive gel silicone implants. Both types of implants have their advantages, and I am happy to accommodate my patients' preferences about implant type. However, overall, most women in New York City tend to prefer traditional highly cohesive silicone gel implants over gummy bear implants.

Gummy Bear Breast Implants

Gummy bear breast implants are form stable, meaning that, much like a gummy bear, if they are cut in half, they will keep their shape. Gummy bear implants can be round or shaped like a teardrop. They tend to be firmer and harder than traditional silicone implants. 

Traditional Highly Cohesive Gel Silicone Implants

Highly cohesive gel silicone implants also maintain their shape if cut in half, however, they are softer and more bouncy feeling than gummy bear implants. Women who have traditional silicone implants report that they feel like a soft, natural breast at a higher rate than women who have gummy bear implants.

Gummy bear implants versus silicone implants

Think of a gummy bear implant like a jelly bean and a traditional silicone implant like a cube of Jell-o. Both are soft. Both are form stable. Both will maintain their shape if cut in half. But a gummy bear implant will feel firmer and be less squeezable, while a traditional silicone implant will be softer and more pliant.

For this reason, most women in my practice (and most women surveyed overall) report a preference for traditional silicone implants.
